Liability waiver

I understand that participation in baseball instruction, training, drills, and related physical activity involves inherent risks, including the possibility of injury from physical exertion, equipment, other participants, playing surfaces, or accidental contact.

I voluntarily authorize the named player to participate in JWH Elite Baseball activities and accept the risks normally associated with baseball training. I confirm that the player is physically able to participate and that I will disclose relevant injuries, limitations, or medical conditions before the lesson.

To the fullest extent permitted by law, I release and hold harmless JWH Elite Baseball LLC, Jake Hager, its coaches, facilities, and representatives from claims arising from ordinary risks connected to participation, except where prohibited by law.

I authorize reasonable emergency assistance if needed and understand that I remain responsible for medical expenses. I also agree to follow facility rules and coaching instructions.

By checking the waiver box during booking, I confirm that I am the player’s parent or legal guardian, or that I am an adult participant, and that I have read and voluntarily accept this agreement.
